ADOS-2 and ADI-R Training Compared: Cost, Duration and Who Can Enrol

If your service is moving into autism assessment, these are the two instruments you will be asked about. They are not alternatives to each other, and choosing between them first is usually a mistake.

Here is what each costs, how long each takes, who can enrol, and how to decide which you need first.

What Each Instrument Does

  • ADOS-2: a structured observation of the person in front of you, now. Social communication and interaction, directly observed and coded
  • ADI-R: a structured interview with a parent or caregiver, covering early development and how difficulties changed over time

One looks at the present, the other at the history, and a defensible diagnosis usually rests on both. That is why services doing this properly tend to train people in both rather than picking a side. It is also why the question “which is better?” does not really have an answer, they are answering different questions about the same person.

What They Cost

Our ADOS-2 Specialist Certification is £1,295 and our ADI-R training is £1,235. UK pricing elsewhere sits broadly in the same range, with instrument publishers and independent trainers both active in the market, so a quote far outside it is worth asking about rather than dismissing.

Budget separately for the instrument materials themselves, which are bought from the publisher rather than from a training provider. This catches people out regularly: the course fee and the kit are two different purchases, and the kit is not optional. Factor it in before you present a figure to whoever holds the budget.

How Long Each Takes

  • ADOS-2: two days, 14 hours, delivered live online
  • ADI-R: two days, 8 hours CPD
  • Post-course work: additional, and the part people underestimate

Who Can Enrol

Both are restricted instruments. Eligibility is set by the publisher rather than by the training provider, and it is based on your qualification level and your role in diagnostic assessment. Typically that means clinical and counselling psychologists, psychiatrists and SAS doctors, speech and language therapists, occupational therapists, and specialist nurses working within an agreed scope.

Check the publisher's current criteria before booking anything. The restriction governs buying the materials as well as attending the course, and it is the single most common reason a booking falls through at the last minute. If you are unsure whether your role qualifies, ask us before you pay - we would rather tell you now than refund you later.

The Distinction That Catches People Out

A certificate of course completion and a certificate of clinical reliability are two different things with two different thresholds. Completion means you attended and passed a post-course assignment. Reliability means your coding agrees with a trained standard, which is a substantially higher bar and a separate process with its own timeline.

Some services accept completion and never ask for more. Others, particularly those under audit or contributing to research, will ask for reliability as a condition of letting you assess. Find out which your service expects before you pay, because the gap between the two is months of additional work and usually a second fee.

What the Training Days Actually Involve

Both courses are taught live rather than as recorded modules, which matters for instruments that are scored on judgement. You work through coded examples, discuss where reasonable clinicians disagree, and practise the scoring decisions that cause most of the error in real use.

The ADOS-2 days include module selection, which is where new assessors most often go wrong. Choosing by age rather than by expressive language level undermines everything downstream of it, and it is easier to get right with someone watching than from a manual.

Which One First?

  • Your team takes a strong developmental history but judges presentation by impression → ADOS-2
  • Your team observes well but relies on patient recall for history → ADI-R
  • Building a pathway from scratch → plan for both, sequenced by which clinicians you have
  • Working single-handed → check the guidance first; autism diagnosis is normally multidisciplinary

Can You Work With Only One?

Sometimes, within a team where someone else covers the other half. Not usually alone. If your service intends to diagnose autism on observation alone, that is worth challenging before you invest in training.

Why Train With Us Rather Than the Publisher

Publishers run excellent instrument training and will always be the authority on their own tool. What they do not do is place the instrument inside a whole neurodevelopmental pathway.

We train in both instruments and in ADHD, which matters because the two conditions co-occur frequently. An autism assessor who cannot recognise ADHD will miss a large minority of cases — and that is a clinical problem, not a marketing point.

What to Budget in Total

  • Course fee for the instrument you need
  • Instrument materials from the publisher
  • Reliability work, if your service requires it
  • Supervised practice before you assess independently

Set against that, the thing worth weighing is how often you will use it. Instrument training is a poor investment for a clinician who will assess twice a year, because coding accuracy decays without practice and a lapsed skill helps nobody. If your caseload will not sustain regular use, it is usually better to train one person properly than three people occasionally.

How to Decide This Week

Ask your clinical lead two questions: does our pathway evidence current presentation and developmental history, and which of those two is weaker? The answer names your instrument. Then ask whether the service requires reliability or accepts completion, because that decides your timeline and roughly a third of your total cost.

Thinking about training with us? Our ADOS-2 Specialist Certification is £1,295 over two days, and ADI-R training is £1,235. The Observership Academy adds observed practice with real patients.

See dates and book on the course pages, or email info@globaladhdnetwork.com and we will tell you honestly whether it is the right course for your role.

Conclusion

ADOS-2 and ADI-R cost around £1,300 each, take two days each, and are restricted to clinicians working in diagnostic assessment. Which you need first depends entirely on what your pathway already does well.

If you tell us how your service currently assesses autism, we will tell you which of the two closes your gap - and whether you need both.
