Job responsibilities
The Pharmacist is responsible for managing a case load of service users that have an established diagnosis and treatment plan for ADHD.
This role provides a unique opportunity to manage a caseload of patients (children, adolescents and adults) with ADHD, ideally working as an independent prescriber, to provide support and advice to patients, carers and families. You will work closely with the ADHD multidisciplinary team and provide specialist pharmacy advice and support to ensure safe, appropriate and clinically effective use of medicines in line with national guidance.
An independent prescribing qualification is essential, and a relevant post-graduate qualification in clinical pharmacy or equivalent experience is desirable.
You will support the development and implementation of evidence-based pathways, audits, policies, procedures, treatment protocols and guidelines that promote patient safety and reduce the risk of harm from medications.
You will perform medication reviews and clinically check prescriptions, ensuring the appropriate, safe and legal supply of medicines, in line with national and local guidelines.
You will log near misses and incidents involving medication and support review and learning where required as follow up
